What is double donation?

Double gamete donation is an in vitro fertilisation (IVF) treatment in which both eggs and sperm used to create the embryos are from donors.

These donors are always anonymous, as established by Spanish law, and are carefully selected according to physical characteristics compatible with the recipient patient or couple.

Once the donated eggs have been obtained, they are fertilised in the laboratory using the ICSI technique. After this process, the generated embryos develop during 5-6 days in the laboratory and the best quality embryo is then transferred into the patient’s uterus, so it can implant and result in a pregnancy.

Thanks to the quality of the donated gametes and rigorous medical monitoring, this treatment has very high clinical pregnancy rates, even in women who have undergone several failed IVF attempts.

Success rates

The success rates of IVF treatment with double gamete donation depend on the individual circumstances of each patient. However, with the relevant diagnostic tests and good follow-up, the following success rates can be achieved with double donation:

First diagnostic visit

Fresh cycles in egg donation. Donor/recipient synchronisation

The Vida Fertility team has over 10 years’ experience in developing synchronised egg donation cycles with patients from all over the world.

To adjust the treatment dates to your availability, we will prescribe medication that will allow the donor’s cycle to be synchronised with the patient’s cycle.

Endometrial preparation

At the start of her cycle, the patient will begin preparing her endometrium by taking oestrogen. The aim is to achieve a suitable endometrial thickness for embryo implantation.

At the same time, the donor will begin ovarian stimulation to enhance follicular growth and obtain a good number of oocytes.

Preparation of the semen sample

On the day of ovarian puncture to obtain the donated eggs, the donor’s semen sample is thawed. According to Spanish law, this sample must have been frozen for at least 6 months to ensure its safety.

Once thawed, the embryology team carefully processes it in the laboratory to select the sperm with the best motility and quality, which will be used to fertilise the eggs using the ICSI technique.

Fertilisation and embryo development

The mature oocytes obtained are fertilised in the laboratory with a donor’s semen to obtain embryos. As a general rule, fertilisation is performed using the ICSI technique of sperm microinjection to increase fertilisation rates. However, conventional IVF can also be used.

The embryos obtained are left under observation in the laboratory incubators until they reach the blastocyst stage (day 5 or 6 of embryonic development).

Embryo transfer

Embryo transfer is preferably performed on day 5 of embryo development, thus increasing implantation rates. It is a simple procedure that consists of placing the selected embryo in the patient’s uterus without the need for any type of anaesthesia. Embryo selection follows very strict quality criteria with the aim of achieving pregnancy in as few attempts as possible.

Who is double gamete donation suitable for?

Double gamete donation is a recommended option when it is not possible to use either the patient’s own eggs or sperm in an in vitro fertilisation treatment. It is indicated in very specific medical and personal situations, such as:

  • Single women or couples of women who wish to start a family with donor gametes. This treatment is ideal for single parents or couples of women seeking the highest success rates from the first attempt.
  • Cases of poor egg and sperm quality, such as in situations of azoospermia (absence of sperm), severe sperm abnormalities (oligoasthenozoospermia), very high sperm DNA fragmentation test, premature ovarian failure or menopause, whether natural or induced by treatments such as surgery, chemotherapy or radiotherapy.
  • When there have been repeated failures in previous IVF treatments, either due to low ovarian response or repeated embryo implantation failures with no apparent cause.
  • Presence of genetic or chromosomal abnormalities in one or both partners that cannot be resolved through preimplantation genetic diagnosis (PGT-A). In these cases, resorting to donor gametes ensures greater safety for the future baby.

Yes. Double gamete donation is fully permitted and regulated in Spain by Law 14/2006 on assisted human reproduction techniques. This legislation guarantees the safety of the process, the anonymity of donors and the rights of people born through these techniques.

Can I find out the identity of the donors?

No. In Spain, egg and sperm donation is completely anonymous. This means that patients cannot know the identity of donors, nor vice versa.

How are donors selected? What tests do they undergo?

In Spain, donor selection is regulated by Law 14/2006 on assisted human reproduction techniques, which is very strict and ensures that the entire process is carried out according to well-defined medical, ethical and legal criteria.

Up to what age can fertility treatment be performed in Spain?

Although Law 14/1006 on Assisted Human Reproduction in Spain does not establish a maximum age for fertility treatments, age is a decisive factor due to the increased risks for the mother and baby.

For women, the generally accepted age for procedures such as IVF in specialised private clinics is up to 50 years old, although this may differ depending on the quality of the patients’ gametes, the regulations of each country and the guidelines of each clinic.

In men, although the ability to procreate is not severely limited by age, sperm quality tends to decline over time.

It is essential to assess each situation individually, taking into account the patient’s overall health, the quality of the eggs or sperm, and other risk factors.
