The assisted reproduction patient is unique

In the field of assisted reproduction, we must treat each patient as a unique case and reach a diagnosis as personalized as possible. Therefore, the first consultation is one of the most important steps for the success of the treatment.

During the first assisted reproduction consultation, our specialists will establish a detailed history of the patient, to better understand her background. Here all details will be important and will play a role in establishing a protocol. During this first appointment, everything will be checked: personal, family, gynecological history, previous treatments and protocols followed (if any), allergies, lifestyle…

In addition, the first consultation will also be an opportunity to perform diagnostic tests and further evaluate the patient’s fertility. Here it will be important to know what tests have already been performed, and to be able to refer the patient for further tests to try to explain previous failures in assisted reproduction.

In-depth diagnostic tests

When we have cases of repeated implantation failures or cases of unexplained infertility, it will be important to try to further study the patient or couple and correct the new protocol to be established.

There are many in-depth diagnostic tests and they will depend on the history of each patient. Not all tests will be appropriate for all patients, and the appropriate diagnostic strategy will need to be identified.

Among the diagnostic tests proposed in cases of implantation failure or repeated miscarriages in assisted reproduction, our specialists may suggest the following:

  • Seeking the ideal moment for embryo transfer through an endometrial receptivity test. Implantation failures, with good quality embryo transfers, can be explained by the fact that the endometrium is not primed with a “typical” endometrial preparation protocol. With a preparation adapted and calculated according to the patient’s window of implantation, we can improve the diagnosis of embryo implantation.
  • Identifying possible defects in blood coagulation and, therefore, in the irrigation of the uterus, with a thrombophilia evaluation.
  • Establishing an immunological evaluation of the patient or the couple, and also of the endometrium, and thus avoiding the rejection of the embryo by the natural defense system.
  • Visualizing possible physical obstacles for implantation through an exploration of the uterine cavity (hysteroscopy).

Different protocols to increase the chances of pregnancy

In assisted reproduction, there are different treatments and different protocols that are decided based on the patient’s history and the chances of success of each treatment.

It is quite common that a patient comes to us for a specific treatment and, after reviewing her history, we refer her to a different treatment. It is important that we explain to each patient the chances of success with one treatment or another and that we provide the information in a transparent and honest way.

Once the treatment has been identified, whether it is insemination, in vitro fertilization or oocyte donation treatment, the protocol to be used must be established.  To do this, the results of the fertility tests and the protocols already used in the past will have to be taken into account: it is all about not repeating the same protocol that did not work, without changing anything.

For example, in IVF treatment, the hormones chosen will be important and each woman will react differently to the doses and drugs. There are short protocols, long protocols, gentle stimulations and also protocols with embryo accumulation (over several stimulation cycles).

In addition, the protocol itself should be complemented with the necessary treatments to favor embryo implantation and the proper development of the pregnancy.
