Dr. Alejandra García-Villalba , Why did you choose to specialise in Assisted Reproduction?
Since the beginning of my career as a physician, I knew I wanted to dedicate myself to gynaecology and obstetrics, but it was during my residency that I discovered my true passion for reproductive medicine.
I am fascinated by how two cells can give rise to an embryo that, when it develops in the womb, becomes a person. This process, which encapsulates the miracle of life, is incredible to me.
Being part of this beginning and helping to create life is a privilege and an experience that fills me with professional and personal satisfaction.
Dr. Alejandra García-Villalba, how has Reproductive Medicine advanced since you entered the world of work?
Advances in reproductive medicine have been spectacular, and every day new research emerges that allows us to improve success rates and personalised treatments even more.
In terms of technology, we have tools such as time-lapse incubators and artificial intelligence, which optimise embryo selection.
Biopsies at advanced stages, such as the blastocyst, allow us to analyse the embryo chromosomically. In addition, new culture media with cytokines improve the chances of implantation, and advanced techniques such as sperm selection help us to choose the best spermatozoa.
In diagnosis, we perform much more complete tests, such as endometrial biopsies, immunological analyses, and specific studies such as sperm fragmentation or sperm apoptosis. Even innovative treatments such as platelet-rich plasma have been shown to be effective in improving endometrial growth and in cases of low ovarian reserve appear to increase follicular activation.
However, there is still much to be discovered, and this constant evolution is what makes this field so exciting.
What surprises you most about being a fertility specialist?
Without a doubt, the miracle of life. Despite years of experience, it never ceases to thrill me to see how an embryo I held in the lab turns into a healthy baby.
It is indescribable when families return with their children to present them to us. Being part of that special moment remains one of the greatest satisfactions of my work.

Dr. Alejandra García-Villalba, is there any branch of assisted reproduction that particularly interests you?
Although I enjoy all aspects of reproductive medicine, in recent years I have worked intensively in cases of low ovarian reserve and in the management of implantation failure and repeated miscarriages.
I am passionate about these challenges because they require a very personalised approach and constant learning to offer solutions to the most complex patients.
What diagnostic test would you recommend to your patients?
Each case is unique, so I always recommend a personalised assessment.
For example, in women with repeated miscarriages or advanced maternal age, chromosomal analysis of the embryos (PGT-A) is key to increase the chances of success.
In cases of implantation failure, we explore the status of the endometrium by hysteroscopy or endometrial biopsy. This individualised approach allows us to identify the specific needs of each patient and design an effective treatment.
In your opinion, what is the most important factor in increasing the chances of success in fertility treatment?
Successful fertility treatment is all about personalisation of treatment. Each patient is unique, and tailoring the plan to her needs, in combination with an expert medical team and the latest technological advances, makes all the difference. In addition, emotional support and empathy are essential to make the process as bearable as possible.
Dr. Alejandra García-Villalba , What would you say to women who are thinking of postponing motherhood?
If you are considering postponing your maternity, it is essential to get information. I would recommend going to a specialised clinic to evaluate your ovarian reserve, detect possible pathologies and explore options such as egg vitrification.
As you get older, your ovarian reserve diminishes and the likelihood of chromosomal alterations increases, which can make pregnancy difficult or increase the risk of miscarriage. Although techniques such as oocyte donation may be a solution in the future, it is also important to be aware of the risks associated with pregnancy at an advanced age. Therefore, having clear information and a multidisciplinary team to guide you is essential to make the best decision.
Dr. Alejandra García-Villalba , What advice would you give to patients who are starting their fertility treatment?
My advice is to choose a clinic that offers clear information, personalised treatments and a committed team that works with empathy. It is important to remember that, although the road may be longer than they imagined, most patients succeed in fulfilling their dream of starting a family.
